After properly filling out all relevant fields on the label:
1. Be sure correct customer number is written on peel-off label, then affix sticker inside the blue square on the front of the box.

2. Place the WHITE copy into the envelope to HAND TO the driver. Do not place inside of or affix to box.

3. Retain the YELLOW copy for your records.
